ABSTRACT

Introducción. El edema pulmonar por reexpansión es una complicación poco frecuente, secundaria a una rápida reexpansión pulmonar posterior al drenaje por toracentesis o toracostomía cerrada. Al día de hoy, se ha descrito una incidencia menor al 1 % tras toracostomía cerrada, con mayor prevalencia en la segunda y tercera década de la vida. Su mecanismo fisiopatológico exacto es desconocido; se ha planteado un proceso multifactorial de daño intersticial pulmonar asociado con un desequilibrio de las fuerzas hidrostáticas. Caso clínico. Presentamos el caso de un paciente que desarrolló edema pulmonar por reexpansión posterior a toracostomía cerrada. Se hizo una revisión de la literatura sobre esta complicación. Resultados. Aunque la clínica sugiere el diagnóstico, la secuencia de imágenes desempeña un papel fundamental. En la mayoría de los casos suele ser autolimitado, por lo que su manejo es principalmente de soporte; sin embargo, se han reportado tasas de mortalidad que alcanzan hasta el 20 %, por tanto, es importante conocer los factores de riesgo y las medidas preventivas. Conclusión. El edema pulmonar de reexpansión posterior a toracostomía es una complicación rara en los casos con neumotórax, aunque es una complicación que se puede presentar en la práctica diaria, por lo cual debe tenerse en mente para poder hacer el diagnóstico y un manejo adecuado.


Introduction. Re-expansion pulmonary edema is a rare complication secondary to rapid pulmonary re-expansion after drainage by thoracentesis and/or closed thoracostomy. As of today, an incidence of less than 1% has been described after closed thoracostomy, with a higher prevalence in the second and third decades of life. Its exact pathophysiological mechanism is unknown; a multifactorial process of lung interstitial damage associated with an imbalance of hydrostatic forces has been proposed. Clinical case. We present the case of a patient who developed pulmonary edema due to re-expansion after closed thoracostomy, conducting a review of the literature on this complication. Results. Although the clinic suggests the diagnosis, the sequence of images plays a fundamental role. In most cases, it tends to be a self-limited disease, so its management is mainly supportive. However, mortality rates of up to 20% have been recorded. Therefore, it is important to identify patients with major risk factors and initiate preventive measures in these patients. Conclusions. Re-expansion pulmonary edema after thoracostomy is a rare complication in cases with pneumothorax; however, it is a complication that can occur in daily practice. Therefore, it must be kept in mind to be able to make the diagnosis and an adequate management.

ABSTRACT

RESUMO Os Problemas de Saúde Mental (SM) e o uso indiscriminado de psicofármacos são problemas de grande relevância para a Atenção Primária à Saúde (APS) e a saúde pública. O objetivo deste ensaio é apresentar uma fundamentação atualizada da tese de Robert Whitaker, desenvolvida no livro 'Anatomia de uma epidemia: pílulas mágicas, drogas psiquiátricas e o aumento assombroso da doença mental'. É apresentada uma síntese do livro, acrescida de comentários sobre determinados temas, visando à melhor ancoragem científica dos argumentos. A tese defendida é que se deve evitar prescrever o uso de psicofármacos; e, caso seja iniciado o uso, que seja como sintomático agudo pelo menor tempo possível. Os argumentos giram em torno de que há evidências favoráveis apenas para redução de sintomas, para algumas dessas drogas e para curtos períodos de uso. Com seu uso crônico, há piora em longo prazo quanto à estabilidade, autonomia e funcionalidade social, com problemas graves de abstinência. Especialmente na APS (e também nos serviços especializados em SM), os profissionais deveriam ter uma abordagem mais crítica dos psicotrópicos e investir em outras abordagens terapêuticas, para fazerem algo melhor, menos iatrogênico e tão ou mais eficaz para os pacientes com problemas de SM no longo prazo.


ABSTRACT Mental Health (MH) issues and the indiscriminate use of psychotropic drugs are a great deal of a problem for the Primary Health Care (PHC) and public health. The aim of this article is to show an updated basis from Robert Whitaker theses in his book 'Anatomy of an epidemic: magic bullets, psychiatric drugs and the astonishing rise of mental illness'. It is presented a synthesis of the book, with specific comments about some topics, aiming for better scientific base of the arguments. The thesis endorses that prescribing psychotropic drugs must be avoided; and, if required, it must be as an acute symptomatic scenario for the least time as possible. The study has positive evidence that a few of these drugs only reduces symptoms, for a short period of time. If chronic used, in a long-term scenario, it seems to actually reduce stability, autonomy and social functionality, leaving the user with serious abstinence from the drug. Specially in PHC (and also in MH specialized services), professionals should have a mindful and discerning approach to psychotropic drugs, and invest in other therapeutic strategies, in order to do something better, less iatrogenic and as effective or more for the mental health patients in the long term.

ABSTRACT

As irregularidades menstruais representam uma série de desordens na quantida- de, duração, frequência ou regularidade do sangramento uterino. Entre suas cau- sas destaca-se o sangramento secundário ao uso de anticoncepcionais, uma razão frequente de descontinuidade dos contraceptivos, podendo aumentar as taxas de gestações não planejadas. Boa parte dos contraceptivos pode levar a mudanças no padrão de sangramento uterino, e a abordagem inicial do sangramentos irregula- res inclui a avaliação de outras possíveis causas, o reforço do uso correto da medi- cação, a tranquilização da paciente quanto à benignidade do quadro e à tendência a melhora com a continuidade do uso. Os anti-inflamatórios podem ser usados como estratégia inicial, e, não havendo resposta satisfatória, há alternativas espe- cíficas para cada método. Este trabalho visa identificar as recomendações atuais sobre o manejo do sangramento anormal decorrente de contraceptivos, por meio de revisão narrativa de estudos publicados sobre o tema nos últimos vinte anos.


Abnormal uterine bleeding represents a series of disorders in the amount, du- ration, frequency and or regularity of uterine bleeding. Among its causes, uterine bleeding secondary to the use of contraceptives stands out as a frequent reason for contraceptive discontinuity, which could lead to unplanned pregnancies. Most contraceptives can cause changes in the pattern of uterine bleeding, and the ini- tial approach of the abnormal bleeding includes assessing other possible cau- ses, reinforcing the correct use of medication, and reassuring the patient about the benignity of the condition and the tendency to improve with the continuity of the treatment. Anti-inflammatory drugs can be used as an initial strategy, and, if there is no satisfactory answer, there are specific alternatives for each contracep- tive method. This work aims to identify them current recommendations on the management of abnormal bleeding resulting from contraceptives use, through a narrative review of studies published on the subject in the last twenty years.

ABSTRACT

Objetivo: desenvolver e estimar a evidência de validade de um instrumento para avaliar a percepção dos acadêmicos de medicina sobre a prevenção quaternária. Método: Trata-se de um estudo metodológico desenvolvido em três etapas: 1) construção do instrumento para avaliação do conhecimento sobre Prevenção Quaternária a partir de referenciais teóricos; 2) evidência de validade baseada no conteúdo, realizada por 13 juízes; 3) evidência de validade baseada na estrutura interna, realizada por 180 acadêmicos de medicina. Análises fatoriais foram realizadas para verificar a estrutura hierárquica do modelo a partir das duas dimensões inicialmente construídas: Domínio Conhecimento e Domínio Prático. Resultados: O coeficiente de evidência de validade de conteúdo da escala total (CVCt) foi de 0,98, demonstrando uma alta concordância entre os avaliadores com a conceituação teórica subjacente. Quanto à precisão do instrumento, todos os itens apresentaram valores de Alfa de Cronbach acima de 0,7, indicando boa precisão. Verificou-se bons indicadores de precisão para a escala com dois fatores (Domínio Conhecimento e Domínio Prático) ƛ2= 109.746, gl= 103; p<0,001; ƛ2/gl= 0,30; CFI= 0,98 e RMSEA 0,019. Conclusão: o instrumento IP4-15 apresentou as evidências de validade de conteúdo, estrutura interna, precisão e propriedades dos itens adequadas para avaliar o conhecimento e a prática da prevenção quaternária de acadêmicos do curso de medicina. Serão necessários estudos futuros para investigar a aplicabilidade do IP4-15 para outras populações.(AU)


Objective: to develop and estimate the evidence of validity of an instrument to assess the perception of medical students about Quaternary Prevention. Method: this is a methodological study carried out in three steps: 1) construction of the instrument to assess knowledge about Quaternary Prevention based on theoretical references; 2) content-based evidence of validity, performed by 13 judges; 3) internal structure-based evidence of validity, performed by 180 medical students. Factor analyzes were performed to verify the hierarchical structure of the model based on the two dimensions initially constructed: Knowledge Domain and Practical Domain. Results: The total scale content validity evidence coefficient (CVCt) was 0.98, demonstrating a high inter-rater agreement with the underlying theoretical conceptualization. As for instrument precision, all items had Cronbach's Alpha values above 0.7, indicating good precision. Good precision indicators were found for the scale with two factors (Knowledge Domain and Practical Domain) ƛ2= 109,746, gl= 103; p<0.001; ƛ2/gl=0.30; CFI=0.98 and RMSEA 0.019. Conclusion: the IP4-15 instrument presented adequate evidence of content validity, internal structure, precision, and properties of the items to assess the knowledge and practice of Quaternary Prevention of medical students. Future studies will be needed to investigate the applicability of IP4-15 to other populations.(AU)

ABSTRACT

ABSTRACT Post-thoracotomy paraplegia after non-aortic surgery is an extremely uncommon complication. A 56-year-old woman presented with a 1-year history of progressive shortness of breath. Computed tomography revealed a locally advanced posterior mediastinal mass involving the ribs and the left neural foramina. Tumor excision with a left pneumonectomy was performed. Post-resection, bleeding was noted in the vicinity of the T4-T5 vertebral body, and the bleeding point was packed with oxidized cellulose gauze (Surgicel®). Postoperatively, the patient complained of bilateral leg numbness extending up to the T5 level, with bilateral paraplegia. An urgent laminectomy was performed, and we noted that the spinal cord was compressed by two masses of Surgicel® with blood clots measuring 1.5 × 1.5cm at T4 and T5 levels. The paraplegia did not improve despite the removal of the mass, sufficient decompression, and aggressive postoperative physiotherapy. Surgeons operating in fields close to the intervertebral foramen should be aware of the possible threat to the adjacent spinal canal as helpful hemostatic agents can become a preventable threat.

ABSTRACT

A crescente produção de iatrogenias justifica a discussão acerca da incorporação da prevenção quaternária na prática médica cotidiana. Este estudo identificou e sistematizou a produção de conhecimento sobre prevenção qua-ternária e sua implicação para a prática clínica. Trata-se de revisão sistemática descritiva apoiada na pergunta nor-teadora: "Quais são as evidências produzidas sobre prevenção quaternária e sua implicação para a prática clínica?" Realizou-se o levantamento bibliográfico entre agosto e setembro de 2020, utilizando-se o descritor "prevenção quaternária", nos idiomas inglês, português e espanhol. A análise dos 30 artigos produziu evidências sobre inter-venções médicas potencialmente danosas que justificariam a adoção da prevenção quaternária. O estudo provoca a comunidade médica a refletir sobre a incorporação da prevenção quaternária, visando uma prática assistencial menos iatrogênica, valorizando a medicina centrada na pessoa e os atributos da atenção primária à saúde. The growing production regarding iatrogenesis justifies the discussion about the incorporation of quaternary pre-vention in the daily medical practice. This study identified and systematized the production of knowledge about quaternary prevention and its implications for the clinical practice. This is a systematic descriptive review supported by the guiding question: What evidence has been produced regarding quaternary prevention and its impact on the clinical practice? A bibliographic survey was carried out between August and September 2020 using the descriptor "quaternary prevention" in English, Portuguese and Spanish. The analysis of 30 articles produced evidence on po-tentially harmful medical interventions that would justify the adoption of quaternary prevention. The study invites the medical community to reflect on the incorporation of quaternary prevention, aiming for a less iatrogenic care practice, valuing person-centered medicine and the attributes of primary health care. SUMMARY OBJECTIVE: This study aimed to assess the prevalence of potentially inappropriate medication prescription in hospitalized elderly patients according to the 2019 American Geriatrics Society Beers Criteria. METHODS: This study is a prospective analysis of electronic medical records of elderly patients admitted to the Department of Medicine, Hospital Central da Irmandade da Santa Casa de Misericórdia de São Paulo, between 1 September 2020 and 30 April 2021. RESULTS: A total of 142 patients (85 women and 57 men) with a mean age of 74.5±7.3 years (65-99 years) were assessed. Of these, 108 (76.1%) were elderly (age ≥65 years and <80 years) and 34 (23.9%) long-lived (age ≥80 years). The average length of stay found in the sample was 25.3±28.7 days (between 2 and 235 days), and 102 out of the 140 patients assessed remained in the hospital for up to 29 days. Sixteen drugs considered potentially inappropriate medication were found in the patients' prescriptions, with at least one potentially inappropriate medication having been prescribed to 141 (99.3%) patients. Elderly patients had a mean of 2.57±0.94 potentially inappropriate medication prescribed versus 2.56±0.89 among long-lived patients. The most prescribed potentially inappropriate medication were as follows: regular human insulin as required (85.2%), and omeprazole (73.9%) and metoclopramide as required (61.3%). CONCLUSION: The study sample showed significant percentages of potentially inappropriate medication prescriptions for the elderly admitted to the hospital.

Introdução: É fato que os idosos compreendem um grupo etário que cresce exponencialmente no Brasil. Inevitavelmente, a alta demanda da maioria dessa população por medicamentos e serviços de saúde acarreta impacto nas políticas de saúde pública no que tange o esforço necessário para garantir o uso racional de medicamentos, evitar iatrogenia e melhorar a qualidade de vida dos idosos. Medicamentos potencialmente inadequados são aqueles que devem ser evitados em idosos, em que o risco de eventos adversos supera o benefício. Métodos: O estudo, de caráter descritivo e retrospectivo e partindo da pesquisa e análise de dados secundários em saúde, objetivou ampliar o conhecimento sobre o impacto do uso de medicamentos pela população idosa, através da coleta de informações sobre as intoxicações por uso de medicamentos disponíveis no site do DATASUS (departamento de informática do Sistema Único de Saúde). Resultados: Em cerca de dez anos relacionados ao período estudado, entre 2010 a 2020, houveram 2.946 internações de idosos causadas por intoxicações farmacológicas, sendo relevante em número de casos as classes dos anticonvulsivantes, sedativos, hipnóticos, antiparkinsonianos. A região com maior número de casos foi a Sudeste. Há diferenças significativas na probabilidade de intoxicação em idosos, sendo maior nos casos de exposição a álcool, a fármacos analgésicos, antipiréticos e antirreumáticos, e a fármacos com ação no sistema nervoso central. Conclusão: Os resultados encontrados advêm da tendência crescente dos problemas associados ao uso de medicamentos por idosos, tornando-se clara a importância de estratégias efetivas de farmacovigilância voltadas a saúde dessa população.


Introduction: It is a fact that the elderly comprise an exponentially growing age group in Brazil. Inevitably, the high demand of most of this population for medicines and health services impacts public health policies in terms of the effort required to ensure the rational use of medicines, avoid iatrogenesis and improve the quality of life of the elderly. Potentially inappropriate drugs are those that should be avoided in the elderly, where the risk of adverse events outweighs the benefit. Methods: The study, of descriptive and retrospective nature and based on the research and analysis of secondary health data, aimed to broaden knowledge about the impact of the use of medicines by the elderly population, by collecting information on intoxications due to the use of medicines available on the DATASUS (departamento de informática do Sistema Único de Saúde) website. Results: In about ten years related to the studied period, from 2010 to 2020, there were 2,946 hospitalizations of the elderly caused by pharmacological intoxications, being relevant in number of cases the classes of anticonvulsants, sedatives, hypnotics, and antiparkinsonian drugs. The region with the highest number of cases was the Southeast. There are significant differences in the probability of intoxication in the elderly, being greater in cases of exposure to alcohol, analgesic, antipyretic and anti-rheumatic drugs and drugs with action on the central nervous system. Conclusion: The results found show a growing trend of problems associated with the use of medicines by the elderly, making clear the importance of effective pharmacovigilance strategies aimed at the health of this population.

Objetivo: Verificar a ocorrência de eventos adversos como infecção do sítio cirúrgico, perda ou infecção do acesso venoso e quedas em pacientes internados em uma unidade de internação cirúrgica de um hospital de ensino do sul do Brasil. Método: Estudo quantitativo, exploratório-descritivo. Realizado de março a maio de 2019, em uma unidade de internação cirúrgica de um hospital universitário do sul do Brasil, com a utilização de dois roteiros estruturados. Foram incluídos e 128 participantes. Análise das variáveis categóricas representadas pela frequência absoluta e relativa e realizadas 701 avaliações. Resultados: Das 701 (100%) avaliações realizadas nos 128 (100%) participantes, houve eventos adversos em 104 (14,7%). Destes, 98 (14,0%) foram registrados mais de um evento adverso por avaliação. Os eventos adversos foram um (1,0%) queda, um (1,0%) infecção de acesso venoso, 33 (31,7%) infecção de sítio cirúrgico e 69 (66,3%) perda de acesso venoso. Conclusão: A ocorrência de eventos adversos evidencia a necessidade do gerenciamento de riscos e a melhoria da qualidade da assistência ao paciente no perioperatório. (AU)


Objective: To verify the occurrence of adverse events such as infection of the surgical site, loss or infection of the venous access and falls in patients admitted to a surgical inpatient unit of a teaching hospital in southern Brazil. Methods: Quantitative, exploratory-descriptive study. Held from March to May 2019, in a surgical inpatient unit of a university hospital in southern Brazil, using two structured scripts. 128 participants were included. Analysis of categorical variables represented by absolute and relative frequency and 701 evaluations were carried out. Results: Of 701 (100%) evaluations performed on 128 (100%) participants, there were adverse events in 104 (14.7%). these 98 (14.0%) recorded more than one adverse event per evaluation. Adverse events were one (1.0%) fall, one (1.0%) venous access infection, 33 (31.7%) surgical site infection and 69 (66.3%) loss of venous access. Conclusion: The occurrence of adverse events highlights the need for risk management and improving the quality of patient care in the perioperative period. (AU)

Se lleva a cabo de marzo a mayo de 2019, en una unidad de hospitalización quirúrgica de un hospital universitario en el sur de Brasil, utilizando dos guiones estructurados. Se incluyeron 128 participantes. Se realizaron análisis de variables categóricas representadas por frecuencia absoluta y relativa y 701 evaluaciones. Resultados: De las 701 (100%) evaluaciones realizadas en 128 (100%) participantes, hubo eventos adversos en 104 (14.7%). De estos, 98 (14.0%) registraron más de un evento adverso por evaluación. Los eventos adversos fueron uno (1.0%) caída, uno (1.0%) infección de acceso venoso, 33 (31.7%) infección del sitio quirúrgico y 69 (66.3%) pérdida de acceso venoso. ABSTRACT

RESUMEN Los pseudoaneurismas de la arteria humeral son infrecuentes, pero pueden asociarse a complicaciones de alta morbilidad como la isquemia de miembro superior. Comunicamos un caso de pseudoaneurisma humeral en el pliegue del codo, que se presentó como tumor pulsátil con leve disminución de la temperatura y parestesias en la mano homolateral de un año de evolución, debido a una punción arterial inadvertida durante la venopunción para extracción de sangre. Se trató con éxito mediante resección quirúrgica más reconstrucción vascular con bypass húmero-cubital y bypass húmero-radial ambos con vena safena. Se discuten las diversas opciones terapéuticas disponibles para los pseudoaneurismas humerales considerando las características anatómicas y la sintomatología del paciente.


ABSTRACT Brachial artery pseudoaneurysms are rare but can be associated with severe complications as ischemia of the upper extremity. We report a case of a brachial artery pseudoaneurysm in the crease of the elbow presenting as a pulsating mass with progressive growth over the past year. The ipsilateral hand was sightly cold and presented paresthesia. The lesion was due to inadvertent arterial puncture during venipuncture. The pseudoaneurysm was successfully treated with surgical resection and vascular reconstruction with a brachial to ulnar artery bypass and brachial to radial artery bypass with saphenous vein graft. The different therapeutic options available for brachial artery pseudoaneurysms are discussed, considering the anatomic characteristics and patients' symptoms.

Introdução: Perfuração radicular é a comunicação entre as paredes do canal radicular e o espaço periodontal. O tempo, a localização e a dimensão da perfuração são fatores que afetam no prognóstico do dente. Objetivo: Relatar um caso clínico de tratamento de perfuração radicular supraóssea associado a enxertia de tecido conjuntivo em área estética. Relato do caso: Paciente relatou ter sido submetido, há 3 meses, ao tratamento endodôntico do dente #22; porém, esse não foi finalizado. Ao exame clínico, o dente apresentava ausência de dor e presença de fístula na região da gengiva inserida. As imagens radiográficas e tomográficas revelaram imagem sugestiva de perfuração radicular na região vestibular do dente #22, além de área hipodensa/radiolúcida circunscrita ao ápice radicular, conduzindo ao diagnóstico de periodontite apical assintomática associada a perfuração radicular supraóssea. Inicialmente, foi realizado preparo do canal radicular e utilizada medicação intracanal por 21 dias. Posteriormente, foi realizada a obturação do canal radicular e restauração definitiva em resina composta na face palatal. Na mesma sessão, foi realizado o acesso cirúrgico para selamento da perfuração com resina composta, associado a enxertia de tecido conjuntivo no dente #22, para evitar recessão tecidual marginal. Foi observada, após 12 meses, neoformação óssea na região periapical do dente #22, com ausência de dor e preservação da estética na região periodontal. Conclusão: O diagnóstico e o planejamento multidisciplinar são fatores importantes no tratamento de perfurações radiculares, assim como a correta escolha do material selador (AU).


Introduction: Root perforation is the communication between the walls of the root canal and the periodontal space. The time, location and size of the perforation are factors that affect the prognosis of the tooth. Objective: To report a clinical case of treatment of supraosseous root perforation associated with grafting of connective tissue in aesthetic area. Case report: Patient reported that 3 months had started root canal treatment of tooth 22, but was not finalized. At the clinical examination, the tooth presented absence of pain and presence of sinus tract in the region of attached gingiva. Radiographic and tomographic images revealed an image suggestive of root perforation in the buccal region of tooth 22, as well as a hypodense / radiolucent area circumscribed to the root apex, leading to the diagnosis of asymptomatic apical periodontitis associated with supraosseous root perforation. Initially, it were performed root canal preparation and intracanal medication. After 21 days, root canal obturation and composite restoration were performed on the palatal face. In the same session, the surgical access was made to sealing the perforation with composite resin, associated to the grafting of connective tissue in tooth 22, to avoid marginal tissue recession. It was observed after 12 months new bone formation in the periapical region of tooth 22, with absence of pain and preservation of aesthetics in the periodontal region. Conclusion: Multidisciplinary diagnosis and planning are important factors in the treatment of root perforations, as well as the correct selection of materials used to seal root perforations (AU).

Introducción: La etiología de la sinusitis maxilar comprende diferentes causas primarias y secundarias. Siempre es preciso descartar de inicio aquellas causas iatrogénicas consecuencia de intervenciones previas. Objetivo: Realizar una revisión de las diferentes causas de sinusitis maxilar, con énfasis en las causas raras iatrogénicas y en el protocolo de actuación. Presentación del caso: Paciente varón de 60 años que presenta episodios de sinusitis maxilar. Como antecedentes quirúrgicos se había realizado quistectomía maxilar, tratamiento de fístula oroantral secundaria y rehabilitación protésica posterior. Tras estudio radiológico inicial se halló una masa intrasinusal, compatible al tacto con material de impresión dental. Conclusiones: Es importante antes de cualquier rehabilitación protésica y de la toma de modelos confirmar la ausencia de fístula oroantral para evitar la intrusión de material extraño en el seno maxilar(AU)


Introduction: The etiology of maxillary sinusitis comprises a number of primary and secondary causes. It is always necessary to initially rule out iatrogenic causes resulting from previous interventions. Objective: Carry out a review of the different causes of maxillary sinusitis, with an emphasis on the rare iatrogenic causes and the clinical management protocols. Case presentation: A male 60-year-old patient who experiences episodes of maxillary sinusitis. Surgical antecedents include maxillary cystectomy, treatment for secondary oroantral fistula and posterior prosthetic rehabilitation. Initial radiological examination revealed an intrasinus mass compatible to the touch with dental impression material. Conclusions: Before any sort of prosthetic rehabilitation and the taking of models, it is important to confirm the absence of an oroantral fistula, to prevent the entrance of foreign material into the maxillary sinus(AU)

Resumen | Introducción. El deseo de mejorar la apariencia física mediante métodos sencillos y económicos, ha generado la aplicación indiscriminada de sustancias modelantes y, con ello, el surgimiento de la alogenosis iatrogénica, enfermedad cada vez más prevalente en Latinoamérica. Objetivo. Describir las características epidemiológicas y los efectos adversos de las sustancias modelantes en un grupo de pacientes de Cali, Colombia. Materiales y métodos. Se hizo una revisión retrospectiva de las historias clínicas de los pacientes que acudieron a consulta por complicaciones producidas por sustancias modelantes durante un sexenio. Resultados. Se incluyeron 1.322 pacientes, 95,5 % de ellos mujeres. Las edades oscilaron entre los 19 y los 83 años, con una media de 39 años. El sitio anatómico de infiltración con sustancias modelantes con mayor frecuencia de efectos adversos, fueron los glúteos. La asimetría y el aumento del volumen en el sitio infiltrado fueron los signos más comunes, en tanto que el dolor, las alteraciones del ánimo y la depresión o la ansiedad fueron los síntomas más percibidos. El 33,6 % de los pacientes desconocía la sustancia aplicada y el 28,1 % refirió haberse aplicado biopolímeros. La mayoría de estos procedimientos estuvo a cargo de personal sin la debida formación. Conclusiones. Estos pacientes requieren la atención de equipos multidisciplinarios para establecer alternativas de tratamiento que mejoren su calidad de vida. Además, se necesitan la regulación de los establecimientos, y las medidas de vigilancia, inspección y control en la importación y el uso de estas sustancias.


Abstract | Introduction: The desire to improve one's physical appearance through simple and economical methods has resulted in the indiscriminate application of modeling substances. As a result, iatrogenic allogenosis has emerged as an increasingly prevalent disease in Latin America. Objective: To describe the epidemiological characteristics and adverse effects arising from the use of modeling substances in a group of patients from Cali, Colombia. Materials and methods: We conducted a retrospective review of the medical records of patients who consulted for complications arising from the use of modeling substances during a six-year period. Results: A total of 1,322 patients were included of whom 95.5% were women. Patients' ages ranged from 19 to 83 years, with an average of 39 years. The most infiltrated anatomical site showing adverse effects due to modeling substances was the buttocks. The asymmetry and increased volume of the infiltrated site were the most common signs while pain, mood disturbances, and depression or anxiety were the most commonly perceived symptoms. A total of 41.8% of patients ignored what substances they had received, and 28.5% received biopolymers; these procedures were mostly performed by non-qualified personnel. Conclusions: The care of these patients requires multidisciplinary teams to establish treatment alternatives to improve their quality of life. In addition, the regulation of establishments, as well as the surveillance, inspection, and control of the imports and use of these substances should be warranted.

Abstract: Maxillary and mandibular incisors have increased risk for severe orthodontically induced inflammatory root resorption. A patient-related risk factor is aberrant root morphology. This study aimed to assess the frequency of detection of different root morphologies in anterior teeth using dental panoramic tomography (DPT) and long cone periapical radiographs (LCPAs). A retrospective cross-sectional design was used to assess a sample of 50 consecutive pre-treatment radiographic records of patients from a specialist orthodontic practice in Adelaide, Australia. A reference guide was developed that included three previously unreported morphologies: pipette and bent, bent and pointed, bent and blunt. Two trained and calibrated assessors examined each record against the inclusion criteria, then independently assessed each anterior tooth from DPTs and LCPAs to detect the type of root morphology present. Data were analysed using the chi-square statistical test. Radiographic records for 48 patients (48 DPTs and 161 LCPAs) were eligible, with 355 and 426 teeth on DPTs and LCPAs, respectively, included for assessment. Normal root morphology (119 teeth) was commonly observed in DPTs, while bent (154 teeth) was frequently observed using LCPAs. Mandibular incisors often had normal morphology in DPTs but bent in LCPAs. Bent was the most common morphology in maxillary lateral incisors using DPT and LCPAs, although maxillary centrals were mostly normal in DPTs but pointed in LCPAs. Differences using the two image acquisition methods were highly significant (p < 0.01). Aberrant root morphologies are more easily detected in anterior teeth using LCPAs compared to DPTs.

ABSTRACT

La meningitis pospunción es una complicación importante y poco frecuente de la anestesia neuroaxial. Describimos el caso de una paciente que ingresa para inducción del parto. Se realiza técnica espinal-epidural para analgesia del parto. Cursando 48 h de puerperio instala cefalea intensa, fotofobia y fiebre. No focalidad neurológica. Sin rigidez de nuca. Al examen, restos cavitarios que impresionan fétidos. Se plantea endometritis puerperal iniciando tratamiento antibiótico. Dado la persistencia del cuadro clínico se plantea punción lumbar para confirmación diagnóstica mediante análisis de líquido cefalorraquídeo; siendo éste turbio, por lo cual ingresa a cuidados intensivos con diagnóstico de meningitis aguda. Bacterióloga informa a los 10 días que el cultivo desarrolla estreptococo mitis oralis. La importancia esta dada porque la meningitis puede ser potencialmente devastadora si no se realiza un diagnóstico y tratamiento tempranos, existiendo medidas que se pueden adoptar para prevenir esta complicación.


Post-puncture meningitis is an important and rare complication of neuraxial anesthesia. We describe case of patient who is admitted for induction of labor. A spinal-epidural technique is performed for labor analgesia. During 48 hours of puerperium, she installed intense headache, photophobia and fever. No neurological focus. No stiff neck. On physical exam, cavitary remains that appear fetid. Puerperal endometritis arises starting antibiotic treatment. Given the persistence of the clinical picture, lumbar puncture is considered for diagnostic confirmation by analysis of cerebrospinal fluid; This being cloudy, for which he was admitted to Intensive Care with a diagnosis of acute meningitis. Bacteriologist reports 10 days later develops streptococcus mitis oralis. The importance is given because meningitis can be potentially devastating if early diagnosis and treatment is not performed, and there are measures that can be taken to prevent this complication.

Un cuerpo extraño es un objeto o una estructura que se incluye accidental o intencionalmente en la intimidad de los tejidos orgánicos de un individuo. Puede desencadenar importantes procesos inflamatorios/ infecciosos, dependiendo de su naturaleza, requiriendo en la mayoría de los casos su extracción quirúrgica, con el fin de evitar daños al paciente. Este trabajo tiene como objetivo informar un caso de cuerpo extraño (fragmento de amalgama) incluido iatrogénicamente en la exodoncia de un molar inferior derecho en un paciente que, después de 8 años, optó por hacerse un implante en el área y descubrió la inclusión de este material, siendo necesaria su extracción quirúrgica para la posterior colocación del implante dental en la región. Los autores destacan la necesidad de realizar una minuciosa inspección y toilette de la caja alveolar en el acto de la exodoncia, para evitar incluir cuerpos extraños en el sitio quirúrgico y evitar así, una nueva intervención en el área (AU)


Foreign body is an object or structure included accidentally or intentionally in the intimacy of the organic tissues of individuals. They can trigger important inflammatory / infectious processes, depending on its nature, requiring its surgical removal in most cases, to prevent damage to the patient. This study aimed to report a case of a foreign body (fragment of amalgam) iatrogenically included in extraction of a right lower molar in a patient who, after 8 years, opted to have an implant in the area and discovered the inclusion of this material, requiring its surgical removal for subsequent placement of a dental implant in the region. The authors highlight the need to perform a thorough inspection and toilet of the alveolar box in the act of extraction, to avoid including foreign bodies in the surgical site, avoiding further intervention in the area (AU)

ABSTRACT

Abstract A 72-year-old woman was admitted for acute heart failure. The echocardiography revealed moderate depression of the left ventricular ejection fraction. Coronary disease was excluded by coronarography. Cardiac magnetic resonance showed predominantly left ventricular septal hypertrophy and severe depression of the left ventricular systolic function. There was also a bright, multifocal and patchy late gadolinium enhancement with subendocardial, mesocardial and subepicardial involvement, suggestive of sarcoidosis. Biochemical study, thoracic computed tomography and positron emission tomography were inconclusive for extra-cardiac sarcoidosis. Therefore, an endomyocardial biopsy was performed. The procedure was complicated by the development of complete atrioventricular block, requiring implantation of a cardiac resynchronization pacing device. A few days after device implantation, the patient developed fever. The echocardiography revealed extensive vegetations, and thus the diagnosis of a device-associated infective endocarditis was made. Even though antibiotic therapy was promptly started, the patient ended up dying. Biopsy results revealed lymphocytic myocarditis. This case is paradigmatic because it shows how the etiologic diagnosis of dilated cardiomyopathy can be challenging. Non-invasive diagnostic exams may not provide a definite diagnosis, requiring an endomyocardial biopsy. However, the benefits versus risks of such procedure must always be carefully weighted.

SUMMARY OBJECTIVE To identify the use of Potentially Inappropriate Medications with imminent risk of Cardiovascular and Cerebrovascular Adverse Events (PIM-CCVAE), in addition to the factors associated with a group of elderly individuals undergoing therapeutic care in a Brazilian public service. METHODS A cross-sectional retrospective study conducted at a secondary level service located in Carapicuíba, SP, Brasil. Only elderly individuals (≥60 years) who were treated in one of the outpatient departments were included. The use of PIM-CCVAE was defined based on the PIM-CCVAEs list. In this research, we used descriptive statistics and logistic regression to identify and track possible predictors of MPI use. All statistical analyses were performed using Stata software version 15.1 (Stata Corporation). RESULTS The sample included 233 elderly individuals, with a mean age of 74.9 (± 9.4) years. Of these, 74.2% used at least one PIM-CCVAE, with an average daily intake of 1.3 (± 1) PIM/elderly. The presence of comorbidities, diseases of the circulatory system, polypharmacy, and low to moderate scores in morbidity and mortality were important factors associated with an increased odds ratio for the consumption of PIM-CCVAE. It is also emphasized that the presence of neurological symptoms proved to be a protective factor for this outcome. CONCLUSION Given the clinical severity and imminent risk of CCVAE in the researched group, preventive measures should be instituted to minimize health problems related to medication in the public network.


RESUMO OBJETIVO Identificar o uso de Medicamentos Potencialmente Inapropriados (MPI) com risco iminente de EAM cardiovascular e cerebrovascular (EAM-CC), além dos fatores associados a um grupo de idosos em vigência terapêutica de um serviço público brasileiro. MÉTODOS Estudo seccional e retrospectivo realizado em um serviço de nível secundário localizado em Carapicuíba, SP, Brasil. Incluíram-se, apenas, idosos (idade ≥60 anos) que foram tratados em um departamento de atendimento ambulatorial. O uso de MPI com risco iminente de EAM-CC foi definido empregando a lista PIM-CCVAEs. Nesta pesquisa, apropriou-se da estatística descritiva e de regressão logística para identificação e rastreio de possíveis preditores de uso de MPI. Todas as análises estatísticas foram realizadas usando o software Stata version 15.1 (Stata Corporation). RESULTADOS Incluíram-se, na casuística, 233 idosos, com média de idade igual a 74,9 (±9,4) anos. Destes, 74,2% faziam uso de pelo menos um MPI com risco de EAM-CC, atingindo uma média de consumo diário igual a 1,3 (± 1) MPI/idoso. Verificou-se que a presença de comorbidades, doenças do aparelho circulatório, polifarmácia e score baixo a moderado em índices de morbimortalidade foram importantes fatores associados ao aumento da razão de chances para o consumo de MPI com risco de EAM-CC. Ressalta-se ainda que a presença de sintomas neurológicos mostrou-se como fator protetor para este desfecho. CONCLUSÃO Dada a gravidade clínica e o risco iminente de EAM-CC do grupo pesquisado, medidas de prevenção devem ser instituídas com o intuito de minimizar os problemas de saúde relacionados à medicação na rede pública.

Abstract Objective: To examine the results of various myocardial revascularization techniques in pediatric patients to better understand the strategies for surgical treatment of coronary artery pathologies. Methods: We analyzed 61 publications dedicated to the indications, methods, and results of coronary bypass surgery in children. Due to the small size of this cohort, case reports are also included in our review. Results: The main indications for coronary bypass grafting in children are Kawasaki disease, myocardial revascularization as a necessary procedure during the congenital cardiac surgery, to manage intraoperative iatrogenic damage to coronary arteries, and homozygous familial hypercholesterolemia. The use of internal thoracic arteries as conduits for coronary bypass grafting in children with Kawasaki disease showed significantly better results in long-term functionality compared to autovenous conduits (87% and 44%, respectively, P<0.001). Acute and late coronary events after arterial switch operation for the transposition of the great arteries, anomalous origin of the left coronary artery from the pulmonary artery, and left main coronary artery atresia are the main congenital heart diseases where surgical correction involves interventions on the coronary arteries. Conclusion: The internal thoracic artery is a reliable and durable conduit that demonstrates proven growth potential in children.

Desde uma perspectiva crítica e de enfrentamento à problemática da sobremedicalização, a prevenção quaternária (P4) emergiu como um conceito inovador, propondo alternativas para prevenir os eventos iatrogênicos e otimizar os custos da saúde. O presente ensaio foi elaborado a partir de uma revisão histórico-conceitual da P4 e dos modelos de educação médica, visando compreender como a P4 poderia se constituir como um elemento estratégico para remodelar a educação médica. O artigo aprofunda aspectos conceituais da P4; aborda a educação médica a partir de uma perspectiva histórica; aponta as interligações dinâmicas entre a P4, as práticas de saúde e a educação médica; e, por fim, expõe os motivos pelos quais a P4 pode se tornar um elemento central da educação médica.(AU)


From a critical perspective that tackles the problem of overmedicalization, quaternary prevention (P4) emerged as an innovative concept, proposing alternatives to prevent iatrogenic events and to optimize healthcare costs. This essay provides a historical-conceptual review of P4 and of medical education models, aiming to understand how P4 can become a strategic element to remodel medical education. The article addresses conceptual aspects of P4, approaches medical education from a historical perspective, and shows the dynamic interconnections between P4, health practices and medical education. Finally, it explains the reasons why P4 can become a central element of medical education.(AU)

El presente ensayo se elaboró a partir de una revisión histórico-conceptual de la P4 y de los modelos de educación médica, con el objetivo de entender cómo la P4 podría constituirse como un elemento estratégico para remodelar la educación médica. El artículo profundiza aspectos conceptuales de la P4, aborda la educación médica a partir de una perspectiva histórica, señala las interconexiones dinámicas entre la P4, las prácticas de salud y la educación médica.
